To the Hon. J. D. Breckenridge Judge of said Court:

 

The undersigned Petitioner Martha Emily Churchill respectfully represents to your Honor that the attached instrument of writing purports to be the Last Will and Testament of Eugene Churchill deceased.  That the said deceased departed this life at Canton, Illinois on or about the 8th day of August A. D. 1923, and at the time of his death resided at Canton, in the county of Fulton and State of Illinois.  Your petitioner further represents that the said deceased left the following named heirs-at-law and legatees: (Indicate if any are minors.)

Martha Emily Churchill, Wife, Canton, Illinois

Dorliska Churchill Jenkins, Daughter, Canton, Illinois

Marie Churchill, Daughter, Canton, Illinois

 

And that the above named persons are still the heirs-at-law and legatees.  Your Petitioner further asks that the Last Will and Testament be admitted to probate.

FULTON COUNTY,} ss.                   Martha Emily Churchill being duly sworn, deposes and says that the allegations in the foregoing petition are true, to be best of her knowledge and belief.
